Ulcerative colitis

Ulcerative Colitis (UC), known as Raktatisara in Ayurveda, is a chronic inflammatory condition that affects the colon (large intestine) and rectum. It leads to the formation of ulcers and inflammation in the colon lining, resulting in frequent diarrhea mixed with blood or mucus. UC is considered an imbalance of Pitta and Vata doshas, which affect the digestive system and cause inflammation. Ayurveda focuses on reducing inflammation, healing the gut lining, and balancing the aggravated doshas for long-term management and relief.

Symptoms:

  • Frequent Diarrhea: Persistent loose stools, often with blood or mucus.
  • Abdominal Pain and Cramping: Pain and discomfort, usually in the lower abdomen, which may subside after bowel movements.
  • Rectal Bleeding: Passage of blood during bowel movements, sometimes accompanied by a feeling of urgency.
  • Fatigue and Weakness: Due to blood loss and frequent bowel movements, leading to weakness and tiredness.
  • Weight Loss: Unintended weight loss caused by poor nutrient absorption and frequent diarrhea.
  • Fever: Mild fever may occur during flare-ups of the condition.

Causes:

  • Dietary Factors: Excessive consumption of spicy, oily, or fried foods, along with alcohol and caffeine, can aggravate Pitta dosha and contribute to ulcerative colitis.
  • Stress and Emotional Imbalance: Stress, anxiety, and mental strain are linked to the worsening of symptoms and flare-ups of UC.
  • Genetic Predisposition: Family history plays a role in the risk of developing ulcerative colitis.
  • Autoimmune Reaction: The body’s immune system mistakenly attacks the colon lining, leading to inflammation and ulcers.
  • Imbalance of Doshas: In Ayurveda, UC is associated with the aggravation of Pitta (inflammation and heat) and Vata (irregular bowel movements and pain), disrupting the digestive system and leading to the condition.

Diagnosis:

Ayurvedic diagnosis involves a thorough evaluation of the patient’s lifestyle, diet, emotional state, and dosha imbalances. Practitioners assess symptoms, pulse (Nadi Pariksha), and the digestive system. In modern medicine, colonoscopy and biopsy are commonly used to diagnose ulcerative colitis and assess its severity.

Treatment:

Ayurvedic treatment for ulcerative colitis focuses on pacifying the aggravated Pitta and Vata doshas, promoting healing of the colon lining, and calming inflammation through diet, herbal remedies, and lifestyle adjustments. Treatments include:

  • Herbal Remedies:
    • Kutaja (Holarrhena Antidysenterica): Known for its anti-diarrheal properties, it helps reduce bleeding and inflammation.
    • Bilva (Bael Fruit): Strengthens the colon lining and reduces the frequency of loose stools.
    • Amlaki (Indian Gooseberry): A potent antioxidant that reduces inflammation and soothes the gut lining.
    • Nagkesar: Helps in controlling bleeding and reducing inflammation in the colon.
  • Dietary Adjustments: A Pitta-pacifying diet is essential, focusing on cooling, easily digestible foods like rice, moong dal (split green gram), pomegranate, and coconut water. Spicy, oily, and sour foods should be strictly avoided. Small, frequent meals that are light and soothing to the digestive system are recommended.
  • Panchakarma Therapy: Ayurvedic detoxification therapies like Virechana (purgation) and Basti (medicated enema) help eliminate toxins and balance the doshas. Basti is particularly effective in treating Vata-related issues in the colon, promoting healing and strengthening the digestive system.
  • Lifestyle Changes: Practicing yoga and meditation can help manage stress and anxiety, which are known to trigger UC flare-ups. Regular relaxation techniques, such as deep breathing and mindfulness, help in reducing symptoms and improving overall well-being.
  • Home Remedies: Consuming buttermilk or takra (diluted yogurt) helps in cooling the digestive tract, while ghee (clarified butter) has healing properties for the colon lining. Aloe vera juice can also soothe inflammation.
