Piles

Piles, also known as hemorrhoids, are swollen veins located in the lower rectum and anus. They can cause discomfort, itching, pain, and rectal bleeding. Piles are classified into two types: internal, which occur inside the rectum, and external, which develop under the skin around the anus. While piles can be uncomfortable, they are generally not serious and can be managed through dietary changes, lifestyle modifications, and Ayurvedic treatments that aim to balance the body’s doshas and promote digestive health.

Symptoms:

  • Bleeding: Bright red blood during bowel movements, noticeable on toilet paper or in the toilet bowl.
  • Pain and Discomfort: Especially during bowel movements, due to swollen veins and inflamed tissues.
  • Itching and Irritation: Persistent itching around the anal area, often caused by the irritation of the skin and swelling.
  • Lumps: Swollen lumps near the anus, which may be tender or painful to touch (common with external piles).
  • Mucus Discharge: Occasionally, mucus may be discharged from the anus, leading to irritation.
  • Prolapse: In severe cases, internal hemorrhoids may protrude outside the anus, causing additional pain and discomfort.

Causes:

  • Chronic Constipation: Straining during bowel movements can cause pressure on the rectal veins, leading to the development of piles.
  • Poor Dietary Habits: A low-fiber diet and insufficient water intake contribute to hard stools and constipation, which increase the risk of piles.
  • Prolonged Sitting: Sitting for extended periods, especially on the toilet, puts strain on the anal veins and increases the likelihood of developing hemorrhoids.
  • Pregnancy: Increased pressure in the pelvic area during pregnancy can cause veins in the rectum to swell, leading to piles.
  • Obesity: Excess weight puts additional pressure on the pelvic and rectal veins.
  • Aging: The tissues supporting the veins in the rectum and anus can weaken with age, increasing the risk of piles.
  • Chronic Diarrhea: Frequent bowel movements and loose stools can also irritate the rectal veins and lead to piles.

Diagnosis:

Ayurvedic diagnosis focuses on assessing the patient’s symptoms, lifestyle, and diet to determine the underlying cause of piles. The condition is often linked to an imbalance in the body’s doshas, particularly Vata and Pitta, which govern digestion and circulation. Nadi Pariksha (pulse diagnosis) helps identify dosha imbalances and guides treatment plans. Modern diagnostic methods may include a physical examination, anoscopy, or sigmoidoscopy to evaluate the severity of piles and rule out other conditions.

Treatment:

Ayurvedic treatment for piles focuses on restoring balance to the doshas, improving digestion, and reducing inflammation. The treatment plan may include:

  • Herbal Remedies: Ayurvedic herbs are used to reduce inflammation, improve digestion, and promote bowel regularity. Some common herbs for treating piles include:
    • Triphala: A powerful digestive tonic that promotes regular bowel movements and relieves constipation.
    • Haritaki: Helps soften stools and reduces strain during bowel movements.
    • Arshoghni Vati: A classical Ayurvedic remedy specifically for managing piles, it helps reduce swelling and pain.
    • Aloe Vera: Cooling and soothing, it helps relieve irritation and inflammation in external piles.
    • Neem: Known for its antibacterial and anti-inflammatory properties, it helps in managing infections and discomfort in piles.
  • Panchakarma Therapy: Detoxification treatments such as Virechana (purgation) can help cleanse the digestive tract and balance the doshas, relieving the symptoms of piles.
  • Dietary Recommendations: A high-fiber diet is crucial to ensure soft stools and prevent constipation. Ayurveda recommends including whole grains, fresh fruits, leafy vegetables, and plenty of water to maintain regular bowel movements. Ghee (clarified butter) and buttermilk can also be included to support digestion. Spicy, fried, and processed foods should be avoided to prevent aggravating Pitta and Vata doshas.
  • Topical Applications: Ayurvedic oils and pastes made from ingredients like sesame oil, turmeric, and ghee may be applied externally to soothe inflammation, reduce pain, and promote healing in external piles.
  • Yoga and Exercise: Regular exercise, such as walking, and specific yoga poses like Malasana (Garland Pose), Vajrasana (Thunderbolt Pose), and Pavanamuktasana (Wind-Relieving Pose), help improve digestion and prevent constipation. These practices also enhance circulation in the pelvic area, reducing the risk of developing piles.
  • Lifestyle Modifications: Avoiding long periods of sitting, practicing proper posture during bowel movements, and managing stress through meditation and Pranayama (breathing exercises) are important for preventing and managing piles. It is also important to establish a regular bowel routine and avoid straining during defecation.
